I also work with other insurance providers as an out-of-network provider.  Please contact your provider to confirm that your plan covers speech and language therapy.  In most cases, I will contact the insurance provider to see if pre-authorization is required for your specific plan.  Information will be requested such as member number, name of child, and date of birth.  A copy of the child’s insurance card (front and back) will give most of the information necessary.
